Insurance and Billing Information

• DMI is a participating provider with Medicare and Medi-Cal. We accept Medicare assignment with the patient responsible for a 20% copay.



• DMI is a preferred provider for most PPOs and HMOs.



• As a courtesy to our Patients, DMI will bill all insurance companies directly.



• DMI accepts Workers Compensation and personal injury liens.



• DMI accepts payment by cash, check and all major credit cards, including MasterCard, VISA and American Express.



• DMI can assist your referring clinician's office by submitting a pre-certification request to your insurance carrier, or a Treatment Authorization Request (TAR) to Medi-Cal.



• A 20% discount is given to patients who pay in flil with check, major credit card or cash. An additional 5% discount is given if such payment is made at the time of the office visit. We believe in passing the savings from lower billing costs directly to our patients.
